Breast enlargement, or augmentation mammoplasty, enhances the body contour of a woman who is unhappy with her small breast size and/or shape. Other purposes of this procedure include:
- To correct reduction in breast volume after pregnancy.
- To balance a difference in breast size.
- As a reconstructive technique following breast removal surgery.
Augmentation mammoplasty is done on an outpatient basis in our surgical center, usually under either sedation or “twilight” anesthesia, or general anesthesia. A breast implant is placed under the breast tissue or under the pectoral muscle depending on the specific situation. The incision can be made under the breast, around the nipple or under the arm. Dr. Tjelmeland uses both silicone and saline implants. Saline breast implants are composed of an outer silicone shell and are filled with saline (IV fluid). There are many different styles of implants including a smooth or textured surface, round or “tear drop” shaped, and implants of different projections. Dr. Tjelmeland will discuss these different implant options with you to achieve the best result.
When the implant is placed submuscularly under the breast, some studies suggest that the rate of capsular contracture (thickening of scar tissue surrounding the implant) is slightly lower. There also may be less risk of visible or palpable implant edges and/or wrinkling. However, in some patients it may be appropriate to place an implant above the pectoral muscle (under the breast tissue) in order to achieve the best result possible. Determining Implant Size
Realistic expectations of this procedure are important. Women often think of breast size in terms of bra cup size. If you are currently a size “A” and wish to be a size “C” or “D”, there must be adequate existing breast tissue coverage for the implant. Often the size of implant used and the final cup size is limited by the condition of your breast and skin.
Recovery: What to Expect
You must have someone drive you home and stay with you for 24 hours after surgery. You will experience some pain the first day or two that is easily controlled with pain pills. Some tightness in the breast area is also normal as your body adjusts to the implants.
You can move about freely after surgery. Avoid heavy lifting for 2-3 weeks. Moderate exercise such as walking or stationary biking is allowed after the first week. Vigorous exercise such as jogging or aerobics may be resumed at 3-4 weeks. You will most likely be able to return to work within a few days, but you will need to avoid any strenuous activity for at least a couple of weeks.
During recovery, the breasts will be somewhat swollen and the implant may appear to be a little “high”due to muscle swelling. Within a few weeks your tissues will adjust to the implant and the actual shape and size will become evident.
